In an effort to address this problem, the French Health Network works with various stakeholders and service providers to develop strategies and resources to be implemented in southwestern Ontario.

New! Implementing the Optimal Model of Long-Term Care for Francophones Manual

This implementation manual provides step-by-step instructions for adapting the Optimal Model of Francophone Long-Term Care to seniors living in residential care settings. It includes 70+ tools, templates and resources as well as steps to reflect on how best to adapt to service provider’s own setting. The recommended sequence of activities recognizes the interdependencies of the activities and is presented in a check-list format to facilitate ease of use.

The implementation plan is organized according to the Five Phases of Collective Impact Framework, providing a foundational approach that leverages cross-sectoral collaboration to foster innovations for social change. This framework is relevant to the implementation of the Optimal Model of Francophone Long-Term Care because it provides guidance on how to initiate, manage and sustain community engagement and multi-partnered collaborations targeting complex health and social issue.

The manual provides work plans and fillable tables to support implementation. We suggest that you download an accessible version of this document to be able to save your work and monitor progress.

The ideas and knowledge in this manual were drawn from the Francophone experience but it is our hope that they be extended to other cultural and linguistic groups, thereby fostering a sense of inclusion that benefits all people regardless of their beliefs, preferences, abilities or cultural and linguistic identity.

 

 

 

 

 

 

 

 

A Guide for Planning and Providing Francophone Long-Term Care Services

A Guide for Planning and Providing Francophone Long-Term Care Services is intended for all those involved in planning, advocating for, and providing Francophone health services in Ontario and other provinces. The French Health Network of Central Southwestern Ontario, in partnership with Entité 4, Bendale Acres, the City of Toronto, the Fédération des aînés et des retraités francophones de l’Ontario (FARFO) and Reflet Salvéo, sought to analyse the Pavillon Omer Deslauriers model at Bendale Acres Long-Term Care (LTC) home and a few others from across Canada that have been effective in minority Francophone situations and to develop practical resources to support the adoption of similar models.

The Guide highlights:

  • The main factors for why Francophone seniors are not consistently receiving FLHS
  • The need for French language LTC services in the Greater Toronto area
  • An optimal model, including 6 key attributes and resources
  • Ten steps to implement a Francophone cluster within LTC
  • Five things frontline staff can do to improve linguistic access.

The information and tools are also beneficial for other community and residential care providers serving Francophones as well as seniors from other language minority groups.
